Fit notes

To request a sick note:

• You will usually need to be seen or spoken too by a clinican before you can be issued a fit note for the first time. Please phone or visit the surgery from 8am to arrange a suitable appointment
• If you you are off work for an exteneded period of time with the same condition and require a futher fit note please fill out a sick note request form. The clinican may reuest that you make a futher appointment before any futher notes can be issued.

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
